The responsibilities of the CKP RWB MCM include assisting in the daily oversight and management of the drop-in center programs and services. The MCM efficiently communicates needs of the center to senior leadership (DCKP) and provides feedback to clients when necessary. The MCM is also responsible for program planning for clients, student interns and volunteers and the narrative reporting for the Ryan White Part B grant. The MCM will carry a caseload of up to 45 clients living with HIV and provide comprehensive service planning, goal setting and resource identification on a regular basis as mandated by RWB funding. ROLE AND RESPONSIBILITIES

Provide assistance with oversight of drop in center program for people living with HIV including coordination of meals, support groups and transportation along with other staff.

To consistently monitor the goals and objectives of the RWB grant and to implement changes as necessary to ensure the success of the project.

To provide continual feedback to the Center for Key Populations leadership, as well as, agency leadership about the progress on goals established for Oasis and to participate fully in all team and agency meetings.

To establish and maintain a strong network of community partners and collaborations that will assist the team in enhancing the resources for patients.

To work with the team to operationalize all project activities including but not limited to HIV testing events, social media campaigns, community outreach, community promotion of services, condom distribution, research and dissemination.

To organize meetings on a regular basis between the Middletown Medical team and the Medical case management teams to ensure truly collaborative communication and care.

To provide medical case management for up to 45 clients according to the guidelines of the RWB grant and the HRSA/HAB standards of care.

To plan and implement programming that is consistent with the goals of the HRSA/HAB measures for clients of the drop in center.

QUALIFICATIONS

  • Bachelor's Degree, preferably in Human Services.
  • Maturity, flexibility, compassion, and the ability to work well with vulnerable populations.
  • Understanding of and ability to access community resources.
  • Work experience in healthcare settings, and community resources experience.
  • Effective communication skills, both oral and written, as well as excellent interpersonal skills and organizational skills.
  • Preferred experience in providing crisis intervention, coordination of services, advocacy and community outreach in working with individuals and families.
  • Excellent PC skills with experience in Microsoft Office Suites.
  • Knowledge of HIV/AIDS and Ryan White services.
  • Must have a valid driver's license and a reliable vehicle a plus.
  • Federally Qualified Health Center (FQHC) understanding a plus.
  • Knowledge of HIPAA, and other state and federal regulations governing healthcare practices.
