To be in compliance with their requirements\, please send the following to the Middletown Police Department:\n\n\n $5.00 background check fee for each worker in your booth that will be handling money. Cash or Check accepted. Check made out to the Middletown Police Department.\n Copy of photo id or driver's license for each worker in your booth that will be handling money.\n A VALID copy of your Sales and Use Permit issued by the State of CT\, or your nonprofit paperwork.\n When dropping off or sending in information to the MPD\, please indicate which business you are with\, and which event you will be participating in.\n\nDEADLINES: Vendors must be registered\, and all documents must be submitted to the Middletown Police Department\, by Wednesday\, July 29\, 2020 by 2:00 p.m. This deadline will be strictly enforced. \n\n\n\nTheir address is: \n\nMiddletown Police Department ATTN: Records\n\n222 Main Street\n\nMiddletown\, CT 06457\n\n\n\nReminder: Food Vendors must also contact the City of Middletown Health Department\, 860-638-4960.\n\n \n\nIf you are participating in other City of Middletown events\, you only have to submit annually.
