Job Description

WFC Project REACH is currently hiring Case Managers for its new Transitional Living Program (TLP). The TLP program works to transition unstably housed young adults (18-21 years of age) onto a path of self-sufficiency and independence.

We are currently hiring for 12 hr/wk positions, 24 hr/wk positions, and Per Diem

Case Managers will be responsible for coordinating services and providing support for clients. This includes addressing service and health needs, helping the client develop personal goals towards independent living, selecting program activities, building skills and reinforcing strengths aimed at achieving their goals.


  • meeting at least weekly with clients
  • assisting clients with daily life skills
  • establishing and maintaining regular contact with schools, employers, and significant others involved with the clients
  • maintaining all relevant paperwork
  • attending all pertinent meetings
  • facilitating client intakes, discharges and referrals
  • providing crisis intervention counseling when needed
  • transporting clients as required

Skills required:

  • ability to provide adequate supervision to ensure client support and safety
  • excellent communication and interpersonal skills in order to demonstrate positive rapport with clients, including establishing professional boundaries
  • ability to utilize effective group skills and techniques


  • Associates Degree in Human Services or related field
  • Two years professional experience
  • At least one year experience working with young adults
  • Must have a valid CT Driver’s License and reliable transportation
  • Also required is the flexibility to be available to clients, including nights and weekends


(24 hr/wk position includes generous paid time off benefits)

Job Type: Part-time

Salary: $12.00 /hour
